Boley Centers’ HUD Section 811 and 202/8 Multifamily Project Rental Assistance Program
Qualifications
Boley Centers provides supportive housing for persons with mental health disabilities and extremely-low to very-low income based on HUD’s income limits. The Head of Household must have a severe and persistent mental health diagnosis, which must be verified by a licensed health professional.
Description of Unit
All apartments in this program are unfurnished one-bedroom units. There is a maximum occupancy of two people per apartment. Domesticated, common household pets are allowed. Each apartment is allowed one warm-blooded pet, under 25 pounds, with a $200 pet deposit.
When you reach the top of the waiting list, you will be offered the first available unit at one of our properties listed below. No property location requests or preferences are accepted. If you require a first floor or barrier-free apartment, make sure to indicate that on the application and be prepared to provide medical verification.

APPLICATION INSTRUCTIONS
Please read the application instructions completely and carefully. Do not leave any blanks. Enter, “None” or “N/A,” if the question does not apply to you.
The Head of Household should complete the application. Make sure you have signed and dated the application. All adult members of the applicant household must sign the application. It is important that all information provided on the application is complete and correct. False, incomplete, or misleading information will cause the application to be rejected.
If you have a disability that requires you to need assistance with the application, please notify us to schedule an appointment. You may call 727-821-4819. For applicants who are Deaf, Hard of Hearing, Deaf/Blind, or Speech Disabled, please dial 711 for the Florida Relay Service.
If your application is complete and appears to meet the eligibility (have a psychiatric disability, have a household size of 1 to 2 people, have very low income according to HUD income limits) for the housing program, you will receive a confirmation letter in the mail indicating your name has been placed on the waiting list. Complete applications will be entered on the waiting list in the order received. Once your application is received, it can take up to 30 days to process. Thank you for your patience. Being placed on the waiting list does not guarantee that your household will be offered an apartment. Applications will be processed according to the Tenant Selection Plan available in the Housing Office.
Once you have reached the top of the waiting list you will be required to provide documentation to verify your eligibility. The documentation required includes:
State issued ID, Social Security Card, and Birth Certificate for each household member. Verification of Disability signed by a licensed medical provider. Authorization for criminal background check for each household member. Third party verification of all incomes and assets. Verification of utility service turned on to the apartment in your name.
If an applicant meets the eligibility requirements and provides the necessary documentation, the applicant will be offered a lease. If the applicant refuses the offer without good cause, does not meet eligibility requirements, or does not provide the required documentation, the applicant will be removed from the list. If there is no response to the offer by the applicant, the applicant will be removed from the waiting list.

HUD 811, Permanent Supported Housing provides permanent housing within the community for individuals who are disabled by mental illness and have low income. Residents pay 30% of their adjusted income.
